I have over 6 years’ experience providing support to individuals with special needs of diverse age groups, in a multicultural environment. This experience incorporates early child education, children and adult with either physical/developmental challenges or who have identified behavioural difficulties. These experiences span Early Childhood Education, Addiction and Community Worker, Adult and Children with developmental and physical disability and Senior Citizens. Vast experience with high-risk and complex needs and have been working as a Care Giver and Support Worker with school, addiction and community health with recruitment agencies. In my years of experience as a Primary Support Worker, I have participated with other in a multi-disciplinary team group identifying goals for individualized support plans; use of relationship approach and skills to motivate individuals to meet planned goals; monitor their progress, coordinate and participate in case management updates, and where required, prepare or provide input for reports. I also perform a full range of duties related to rehabilitation and supervision to promote the well-being of individuals with challenging behaviours, physical and mental health challenges. I ensure the safety and security of individuals, staff and community through adherence to all policies and procedures. I support daily routines; participate in unit programs and recreational activities. I report constructive feedback and assistance through written and verbal communication to professional staff, colleagues, and team members and put in place proactive individual programming and interventions strategies that aid good interactions. My knowledge and experience puts me in an advantageous position to ensure service users with challenging behaviours, difficulties or disabilities are provided with care support consistent with their functional abilities. This is done by following relevant policies, procedures and processes consistent with legislative and educational requirements and industry best practices. As part of my responsibilities, I maintain security of the day program and care centre, such as key control, regular formal/informal counts, respond to emergencies such as fire, medical, escape attempts, disturbances, attempts, -related behaviour, and utility disruptions; and prepare written reports for incidents.
